 Claims process

Inzuzo Funeral Plan, cash will be paid out 48 hours after we have received all the required information for a valid claim. The funeral cash pay-out will be either R3 000 or R12 000. You'll have cover from day one for accidental death, with the option of cover for you, your spouse and your biological and/or legally adopted children. If you have parents dependant on you for financial support, the plan can cover them too for an additional premium. And in the event of accidental death of a main member, double cover will be provided. In addition, the plan provides you with funeral support service, legal support service, trauma, assault and HIV treatment, all at no additional cost.

DEATH CLAIMS PROCESS

 

The Benefit Management Unit within Inzuzo and Old Mutual Assurance is responsible for the

efficient and accurate payment of all claims. There are three components in the process, i.e.

submission, validation and payment.

 

  • Submission of a Family Cover claims

 

      Claims should be submitted within 52 weeks after the Insured Person’s death. Premium

      payment should be up to date in terms of the policy contract.

 

      Inzuzo may ask for further information on any aspect of the claim. These additional

      requirements should be met within a period of 26 weeks from the date on which the

      Fund/Employer was notified. If not, the claim will be cancelled unless Inzuzo agrees to

      extend the period.

 

Documents required in respect of a Family Cover claim

 

  • A completed Family Cover Claim form (signature, date stamp and company stamp)

      must be submitted.

  • A certified copy of the death certificate (certified by a Commissioner of Oaths or any

      other person authorised to certify documents) is to accompany the form. The certifier's

      full name, signature, designation, physical address and contact number are also to be

      provided. Should a S.A.P.S. official certify the document, the officer's full name,

      signature, rank, Force Number and location are also to be provided. The words "True

      copy of the original" are to appear on the document.

  • Proof of identity of the claimant (ID document).

  • Proof of relationship

 

Insured Spouse:

 

  • A marriage certificate (certified by a Commissioner of Oaths or any other

      person authorised to certify documents, stating his/her full name,

      designation, physical address and contact telephone number). The words ‘true

      copy of the original’ must appear on the document, OR

  • With regard to a custom marriage or where the couple has been living

       together for more than 6 months (common law situation) a sworn affidavit is required

 

Insured Child:

 

  • A affidavit confirming proof of relationship and/or the birthcertificate.
